Facility: Children's Home Care of Dayton Department: Home Infusion Pharmacy Schedule: Part time Hours: 20 Job Details: The home infusion pharmacist is responsible for providing pharmaceutical care, primarily of an intravenous therapy nature, to home care patients. Consult and coordinate with physicians, nurses, third party payers, caregivers and patients in planning, providing, implementing, and monitoring effective drug therapy. This includes assessing drug orders for accuracy and appropriateness, developing care plans, maintaining medication profiles, monitoring and documenting patients' progress and outcomes, compounding sterile products, and selecting appropriate supplies.
Bachelor’s Degree in Pharmacy or PharmD Degree is required
Rph Ohio - Registered pharmacist - Ohio State Board of Pharmacy is required
