• Pharmacist degree required.

Responsibilities

• Provide pharmaceutical care, primarily intravenous therapy, to home care patients.
• Consult and coordinate with physicians, nurses, third party payers, caregivers and patients in planning, implementing, and monitoring drug therapy.
• Assess drug orders for accuracy and appropriateness, develop care plans, maintain medication profiles, monitor and document progress and outcomes.
• Compound sterile products and select appropriate supplies.
