Role Description

The Pharmacist is a full-time, on-site role based in Waynesboro, VA, responsible for overseeing and coordinating pharmaceutical services for infusion patients. Day-to-day responsibilities include verifying and interpreting prescriptions, performing clinical assessments, and preparing and dispensing sterile and non-sterile medications in accordance with applicable regulations and standards. The Pharmacist collaborates with physicians, nurses, and care coordinators to develop and adjust medication therapy plans, monitor treatment effectiveness, and provide medication counseling to patients and caregivers. This role also maintains accurate documentation, participates in quality and safety initiatives, manages inventory of medications and supplies, and supports compliance with state, federal, and accreditation requirements. The Pharmacist is expected to contribute to continuous improvement of pharmacy operations and to uphold a high level of professionalism, ethical practice, and patient advocacy.

Qualifications

• Active Pharmacist license in good standing in the state of Virginia (or eligibility for licensure), and Doctor of Pharmacy (PharmD) or equivalent degree from an accredited school of pharmacy.
• Experience in infusion therapy, hospital pharmacy, or clinical pharmacy practice, with strong knowledge of sterile compounding standards (e.g., USP 797/800) and infusion-related medications.
• Proficiency in medication verification, dosage calculations, and drug interaction screening, with strong attention to detail and commitment to patient safety.
• Ability to collaborate effectively with interdisciplinary care teams, communicate clearly with patients and caregivers, and provide understandable medication education.
• Comfort using pharmacy management systems, electronic health records, and related technology to document care and manage workflows efficiently.
• Strong organizational and time-management skills, with the ability to prioritize multiple patients and tasks in a fast-paced clinical environment.
• Commitment to evidence-based practice, continuous professional development, and adherence to all regulatory and accreditation requirements.
• Prior experience in home infusion or ambulatory infusion center settings is preferred; board certification or additional clinical training is a plus.
