Job summary

• Active and unencumbered Registered Pharmacist (RPh) license in primary state of practice; willingness to obtain additional licenses.
• 1–2 years of clinical experience in home infusion or specialty pharmacy.
• IgCP and CSP certifications are highly preferred.
• Strong clinical assessment skills, disease state management, and telephonic communication; proficient with EHR and dispensing systems.

Role Overview

We are seeking a dedicated Clinical Pharmacist specializing in Specialty Pharmacy and Home Infusion Therapy to deliver high-touch, patient-centered clinical care. In this role, you will oversee the therapeutic management, clinical monitoring, and care coordination for patients receiving complex, high-cost, and life-sustaining infusion and specialty medications at home. You will work closely with prescribers, home health nurses, and multidisciplinary care teams to ensure seamless treatment transitions, optimal clinical outcomes, and enhanced patient safety.

Key Responsibilities

• Clinical Management & Pharmacokinetics: Conduct comprehensive baseline and ongoing clinical assessments, evaluating lab values, drug interactions, and clinical parameters to manage individualized therapy plans and dosing adjustments.
• Specialty & Home Infusion Oversight: Direct clinical management across disease states including biologics, specialty injectables, immune globulin (IVIG/SCIG), and targeted therapies.
• Care Coordination: Partner with prescribers, specialty clinics, and home health care agencies to schedule therapies, align delivery dates, confirm vascular access plans, and manage therapy transitions.
• Patient Education & Telephonic Care: Provide structured telephonic counseling, onboarding, and ongoing education to patients and caregivers regarding therapy expectations, administration schedules, pump operation, and side-effect management.
• Safety, Outcomes & Accreditation: Monitor for adverse events and therapeutic efficacy while maintaining compliance with specialty pharmacy accreditation standards (e.g., URAC, ACHC) and clinical practice guidelines.

Qualifications & Skills

• Education: Doctor of Pharmacy (Pharm.D.) from an accredited institution.
• Licensure: Active and unencumbered Registered Pharmacist (RPh) license in primary state of practice required. Highly Preferred: Licensure in multiple states (or willingness to obtain additional state licenses via reciprocity) to support regional/multistate patient populations.
• Experience: Minimum 1-2 years of clinical experience in home infusion or specialty pharmacy.
• Certifications: IgCP and Certified Specialty Pharmacist (CSP) highly preferred
• Core Competencies: Strong clinical assessment skills, expertise in specialty disease state management, clear telephonic communication, and proficiency with Electronic Health Records (EHR) and dispensing systems. Experience with CPR+ or CareTend highly preferred.
