Responsibilities/Job Description:
- Ensures that all pharmacy operations, practices and procedures meet all licensing and standards requirements established by local, state and federal agencies and accreditation associations (e.g. TBP, ACHC, URAC)
- Provide appropriate pharmaceutical care including patient counseling and prospective drug use evaluation to patients receiving home IV therapy.
- Provide prescription drug therapy to patients ensuring accurate labeling and dispending of medications in a timely manner
- Provide oversight of the safety, appropriateness, and effectiveness of the pharmaceutical care provided to patients (consumers) as it relates to specialty products dispensed
- Enters prescription orders.
- Demonstrates the ability to interact in a positive and helpful manner with patients, visitors, physicians, and co-workers. Reflects commitment to building a supportive work environment and maintains a positive attitude.
- Prepares inventory, orders supplies, and stocks shelves in the pharmacy, and appropriately uses these resources.
- Monitors for compliance with all HIPAA regulations, adherence to all State and Federal regulations and laws, accrediting bodies (TJC, URAC, etc.), adherence to all infusion pharmacy accepted standards of practice, responsible for handling complaints and / or grievance programs
- Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
Pharmaceutical Care:
- Knowledge of IV antibiotic medications, TPNs, inotrope therapies, specialty medications and strong pharmacy practice skills.
- Knowledge of therapeutic drug levels and common laboratory values
- Knowledge of advanced pharmaceutical calculations
- Documents clinical interventions in a timely and professional manner.
- Monitors and reports medication errors, adverse drug reactions, allergies, and patient adherence issues
- Collaborates with patients, caregivers, and other healthcare professionals
- Reviews patient records to determine the appropriateness of medication therapy and corresponding labs
- Excellent understanding of pharmaceutical therapy and direct patient care
- Proficient understanding of medication administration and health & safety guidelines
- Knowledge of basic pharmaceutical calculations
- Strong leadership qualities with excellent verbal and written communication skills; self-directed with excellent time management
Qualifications:
Unfortunately no new grads (at the moment).
Infusion / sterile compounding experience a must.
- 20 Hour ACPE Approved Sterile Products Certification
- Infectious disease knowledge and pharmacokinetic dosing experience is required
- Minimum 1 year sterile compounding experience
- Prior experience working in Ambulatory, Infusion or Home Health preferred
- Strong attention to detail and ability to work in a fast-paced environment
Scheduled shifts will be Monday - Friday; with alternating call schedules after-hours and weekends.
The Clinical Staff Pharmacist will be working in our Infusion & Specialty Pharmacy located in Houston, TX.
