Job summary

PromptCare is seeking a Home Infusion Pharmacist in Lenexa, KS to review charts and labs for patients on complex infusion therapy. You will collaborate with physicians, nurses, and the care team to ensure accurate, safe medication management and optimal outcomes.

The role involves supervising pharmacy technicians, evaluating orders, and communicating progress with the treating team by phone or in our infusion suite, with a focus on quality, compliance, and patient-centered care.

Qualifications

• B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate.
• Active licensed pharmacist in Kansas; additional states as required.
• 1 year hospital and home infusion therapy experience preferred; two years clinical pharmacy experience preferred.

Responsibilities

• Participate in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
• Review physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
• Monitor patient progress and communicate findings with the physician and healthcare team by phone or in infusion suite.
• Directly supervise pharmacy technicians in compounding, packaging, and labeling of sterile products.
• Assist in training and orientation of new employees and performance improvement activities.

Description

We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care.

Full-Time, 40 hours per week, Mon-Fri 8:00am-5:30pm CST

Requirements

• B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
• Active registered pharmacy license in the state of Kansas and additional states as required without restrictions
• 1 year of hospital and home infusion therapy experience preferred
• Two years of recent clinical pharmacy experience preferred
• Thorough knowledge of infusion therapies preferred
• Recent inventory management experience preferred
• Accuracy in mathematical calculations including pharmacokinetic dosing preferred
• Knowledge of CPR+ software preferred

Job Responsibilities, Included But Not Limited To

• Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management
• Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness
• Monitor the patient’s progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. Most patient contact will be on the phone, and, on occasion, you will meet them in our infusion suite
• Directly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging, and labeling of compounded sterile product
• Assist in training and orientation of new employees, performance improvement activities and educational programs
• Additional licensure may be required in multi-state service areas
• Go in to IV room and mix medications if needed
• Other duties assigned
