Job summary

PromptCare in Lenexa, KS seeks a Home Infusion Pharmacist to review charts and lab results for an assigned caseload of patients on specialized infusion therapy. You will collaborate with physicians, nurses, and the healthcare team to ensure high-quality care and safe medication use.

The role requires a Kansas license and a B.S. or PharmD degree, with 1 year hospital/home infusion experience and 2 years clinical pharmacy experience preferred.

Qualifications

  • Pharmacy degree or equivalent licensure required.
  • Active pharmacist license in Kansas; multi-state eligibility as needed.
  • One year hospital and home infusion experience preferred.
  • Two years of recent clinical pharmacy experience preferred.
  • Strong knowledge of infusion therapies preferred.
  • Recent inventory management experience preferred.
  • Accurate pharmacokinetic dosing calculations preferred.
  • CPR+ software knowledge preferred.

Responsibilities

  • Participate in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
  • Collect, interpret, and evaluate physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
  • Monitor patient progress and communicate clinical information with the physician and healthcare team; most contact will be by phone, with occasional in-person visits.
  • Directly supervise pharmacy technicians and support staff in compounding, packaging, and labeling of sterile products.
  • Assist in training and orientation of new employees, performance improvement activities and education programs.
  • Additional licensure may be required in multi-state service areas.
  • Go into IV room to mix medications if needed.
  • Other duties assigned.

Description

We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Requirements


  • B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate

  • Active registered pharmacy license in the state of Kansas and additional states as required without restrictions

  • 1 year of hospital and home infusion therapy experience preferred

  • Two years of recent clinical pharmacy experience preferred

  • Thorough knowledge of infusion therapies preferred

  • Recent inventory management experience preferred

  • Accuracy in mathematical calculations including pharmacokinetic dosing preferred

  • Knowledge of CPR+ software preferred
