The Opportunity:

WORK SCHEDULE

• A. The Contractor shall provide on-site pharmacist services and coordinate patient care with all appropriate departments.
• B. Normal shifts for the Pharmacy Department consist of 8 hour shifts. Working hours are shifts between the hours of 8:00 am to 5:00 pm with a 1 hour lunch break or 8:00 to 4:30 pm with a half hour lunch break depending on the site assigned, Monday through Friday (7:00 am to 4:00 pm, 9:00 am to 6:00 pm or 1:00 pm to 10:00 pm) and 10:00 am to 6:00 pm Saturday and Sunday (at select facilities). No federal holidays are worked.

PROFESSIONAL QUALIFICATIONS

• A. Graduate of accredited College of Pharmacy
• B. Documentation of annual Continuing Pharmacy Education required for licensure
• C. Shall possess current unrestricted license to practice in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States at all times
• D. Contractor shall submit resume of past professional experience
• E. Documentation of a Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy specialties OR completion of a Post Graduate Year One residency
• F. Demonstrated experience in management of patients with diabetes within the context of a collaborative practice agreement.
