Position Overview

Inpatient pharmacists provide comprehensive medication management for hospitalized patients, focusing on safety, efficacy, and timely therapy.

They participate in rounds, consult with care teams, and support transitions of care within the hospital setting.

Responsibilities
  • Review inpatient orders for accuracy, dosing, and interactions
  • Collaborate with medical teams to optimize regimens and monitor adverse events
  • Participate in multidisciplinary rounds and provide drug information
  • Assist with high-risk therapies and antimicrobial stewardship as appropriate
  • Educate patients and staff on medications and safety measures
  • Document interventions and outcomes in the electronic health record
Qualifications

Education: Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.

Licensure: Licensed pharmacist in the practicing state.

Certifications:

  • Immunization certification; antimicrobial stewardship training preferred

Experience or Skills: Experience in hospital-based clinical pharmacy; strong communication and analytical skills

Experience
