As an PH PART TIME Perinatal Registered Nurse within the Habersham County Health Department. At the District Two, Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways. District 2 Public Health serves 13 counties in northeast Georgia, including Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, and White counties. Job Responsibilities

PH RN 1

Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures

PH RN 2

Under general supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Focuses on the prevention of illness, injury or disability, the promotion of health and maintenance of the health of populations. Cares for population with special needs during disasters/emergencies and biological and chemical protocols.

Additional Duties

• Provide outreach, education, and case management to high-risk perinatal & postpartum women, and infants up to one year of age through home visits.


• Conduct home visits for prenatal, postpartum, and infant assessments to assess patient conditions and coordinate with providers to ensure appropriate care.


• Educate patients, offer referrals for additional services, and document observations and follow up plans.


• Attend required meetings (local, District, and State).


• May supervise additional staff as needed.


• Willing to travel within District 2 as necessary.


• Familiar with OB/GYN hospital practices.


• Other duties as assigned.

Minimum Qualifications

PH RN 1

Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. Note: No experience required.

PH RN 2

Bachelor's degree in Nursing from an accredited college or university AND Current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR

Associate's degree/diploma in Nursing from an accredited college AND One year of PHN experience AND Completion of a PHN course within one year of hire AND Current Georgia Registered Professional Nurse license AND CPR certification.
