Job summary

Senior Community Care of Kentucky PACE in Louisville, KY seeks a Personal Care Supervisor to lead the Home Care and Adult Day Health Center teams, ensuring care meets policies and state/federal regulations. Pay Range: $42,000 - $63,000, Based on experience
Schedule: Monday - Friday, occasional evening and weekends

OBJECTIVE: Under the supervision of the Manager of Home Care, supervises all direct staff in the Adult Day Health Center and the Home Care departments in conformance with policies, procedures, and standards and all governing state and federal regulations. Responsibilities include the planning, coordination, and implementation of the dementia programming for all direct care staff.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Bachelor’s Degree or equivalent education and experience.
  • Current certification with appropriate state in which program operates as Certified Nursing Assistant (C.N.A.).
  • Three (3) years’ experience in a geriatric health setting including a minimum of two (2) years of demonstrated successful supervisory experience. Management experience preferred.
  • Certification or equivalent experience demonstrating knowledge of comprehensive dementia care.
  • Proficiency with various business software, preferably Microsoft Office.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunization before having direct participant contact.

Skills and Knowledge:

  • Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.).
  • Working knowledge of the PACE regulations and state regulations.
  • Working knowledge of healthcare and aging networks.
  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
  • Working knowledge of quality improvement and cost containment systems.
  • Demonstrated effective leadership and decision making skills.
  • Demonstrated strong facilitation and conflict resolution skills. Ability to foster and build professional relationships among staff.
  • Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities.
  • Effective written and oral communication skills. Working knowledge of local health care and geriatric service networks.

ESSENTIAL FUNCTIONS:

  • Staff Management
  • Directs, supervises and evaluates the performance of the C.N.A.s, and P.C.A.s. Completes supervisory visits within scope and in conjunction with Manager of Home Care.
  • In consultation with the Manager of Home Care, Day Center Supervisor and other management staff, assists in developments and implementation of policies, procedures and standards for the Adult Day Health Center and Home Care departments. Communicates changes to staff, monitors outcomes and makes changes as needed.
  • Conducts interviews and recommends candidates for hire.
  • Provides for orientation, ongoing training and competency evaluation of staff according to program needs, and state and federal regulatory requirements. Ensures that the job responsibilities, authorities and accountabilities of all direct reports are defined and understood.
  • Fosters team-building, professional relationships and good communication among staff and other departments. Routinely communicates and implements changes in program goals, philosophy and policy to staff.
  • Identifies the continuing education needs of assigned staff, and collaborates with the Center Director and Director of Clinical Services and Quality and Compliance Manager to establish the most effective and efficient approaches to addressing same.
  • Assists in the development of organizational staffing patterns and position descriptions for assigned staff.
  • In conjunction with Manager of Home Care and Home Care Scheduler, establishes and maintains operational systems to ensure uniform and consistent delivery of care and services provided in the homes of participants.
  • Provides on-going support and training to all staff involved in dementia-specific programming related to the dementia disease process
