Job summary

Volunteers of America National Services (VOANS) in Florence, KY invites you to join Senior Community Care of Northern Kentucky PACE as a Licensed Clinical Social Worker. You will provide compassionate, patient-centered clinical care to residents within a supportive interdisciplinary team.

Requirements include KY LCSW licensure and a Master’s Degree in Social Work. The role emphasizes collaboration, care planning, and promoting residents’ well-being in a facility-based PACE program.

Qualifications

• Must have KY LCSW licensure.
• Master's Degree from an accredited school of social work.
• Strong communication and customer service skills.
• Must have a valid driver’s license and transportation.
• Must clear background check.
• Long-term care experience preferred.

Responsibilities

• Serve as a contributing member to the PACE Interdisciplinary Team (IDT).
• Provide direct clinical care to residents, including activities of daily living such as bathing, dressing, grooming, and mobility.
• Support residents’ physical and emotional well-being by building meaningful connections and delivering compassionate, person-centered care.
• Monitor and report changes in residents’ condition to the nursing team to ensure timely and appropriate care.
• Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards.

Pay Range

$65,000 - $80,000 (Based on experience)

Schedule

Monday - Friday, 8:00am - 5:00pm (Occasional evenings and weekends)

What You Bring (Requirements)

• Current Licensed Clinical Social Worker licensure in KY


• Education: Masters’ Degree from an accredited school of social work required.


• Strong communication and customer service skills


• Ability to follow care plans and resident orders


• Long-term care experience (preferred)


• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.


• Must have a valid driver’s license and have means of transportation.


• Must clear background check.

Senior Community Care of Kentucky PACE (Northern) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs.
