Zusammenfassung

Ohio’s Hospice in Columbus, OH is seeking a Part-Time Personal Care Specialist (CNA/STNA) for daytime hours, approximately 24 hours per week. Shift 6:30 a.m.–7:00 p.m. with every third weekend and holiday rotation.

Inpatient unit at OSU Wexner Medical Center. You will assist with personal care under RN supervision and document care in the electronic health record. The role emphasizes high-quality nursing assistance per the plan of care, with opportunities for certification and career growth

Qualifikationen

• High school diploma or GED is required.
• State-tested Nursing Assistant certification in Ohio preferred; non-certified applicants must complete 75 hours of in-house training.
• Hospice certification (CHPN) preferred and available.
• Ability to document care in electronic health records.
• Valid driver’s license with safe driving record; automobile insurance required.

• Review Plan of Care before each visit to ensure correct care.
• Assist with personal care: bathing, dressing, oral hygiene, meals, transfers, exercises.
• Maintain clean, safe patient environment and monitor safety factors.
• Report vital signs and readings to the RN promptly.

• Communicate patient changes with the RN and interdisciplinary team.
• Precept new staff and support orientation and training.
• Participate in Continuous Quality Improvement activities.

Job Summary/Purpose

Personal Care Specialist provides quality nursing assistance to patients according to the plan of care workingunder the direction of RN.

Qualifications

• High School Diploma or GED
• Certified: Certified State Tested Nursing Assistant with State of Ohio without any board actions
• Non-Certified: CNA not required but must complete the required 75 hours of in-house training upon hire:must meet internal competency checklist
• Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’sHospice employees who wish to obtain certification.
• Computer skills sufficient to properly document services and care
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage

Key Responsibilities

Reviews Plan of Care prior to making each visit to ensure the correct care is given to every patient assigned.

• Provides or assists with personal care, including but not limited to bathing, dressing, oral hygiene, hair care, meal preparation, feeding, transfers and exercises based on plan of care.
• Maintains comfortable clean environment by adjusting lighting, room temperature, changing bed linens and light housekeeping as needed. Alert to safety factors and accident prevention.
• Reports results timely and accurately to the RN for any vital signs, or other equipment readings as ordered by the plan of care.
• Participates as a member of the interdisciplinary team and communicates with the RN any patient changes or updates to ensure proper response time and quality patient care.
• Documents activities carried out into the electronic health record at the point of care.
• Follows individualized plan of care, documenting comprehensive observations, interventions and all communication with supervising RN.
• Recognizes and reports changes in mental or physical status of patient promptly to RN to provide sufficient communication to team members.
• Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio’s Hospice patients.
• Participates in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services.
• Precepts new staff to support professional relationships with newly hired team members as required.
