Job summary

Ohio’s Hospice seeks a Personal Care Specialist (CNA/STNA) for part-time daytime hours in Columbus, OH. The role requires providing nursing assistance under RN supervision and supporting the plan of care in the inpatient unit at OSU Wexner Medical Center.

You will work roughly 24 hours weekly, with a 6:30 a.m. – 7:00 p.m. shift and every third weekend, participating in team care and documentation in the electronic health record.

Qualifications

• High School Diploma or GED is required.
• Certified: Certified State Tested Nursing Assistant with State of Ohio without any board actions.
• Non-Certified: CNA not required but must complete 75 hours of in-house training on hire.
• CHPN certification preferred and may be obtained with employer support.
• Computer skills to document care.
• Ability to drive in day or night and in inclement weather.
• Valid Driver's License with safe driving record.
• State minimum auto insurance coverage.

Responsibilities

• Reviews Plan of Care before each visit to ensure correct care.
• Provides or assists with personal care according to plan of care.
• Maintains a clean, safe environment and monitors safety factors.
• Reports vital signs and readings to the RN promptly.
• Communicates patient changes to the RN and team.
• Documents activities in the electronic health record at the point of care.
• Follows the care plan and documents observations and interventions.
• Reports changes in mental or physical status to the RN.
• Assists other departments to ensure comprehensive patient care.
• Participates in quality improvement activities.
• Precepts new staff as required.

Personal Care Specialist – Ohio’s Hospice

Position: Part-Time

Job Summary/Purpose

• Non-Certified: CNA not required but must complete the required 75 hours of in-house training upon hire:must meet internal competency checklist
• Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’sHospice employees who wish to obtain certification.
• Computer skills sufficient to properly document services and care
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage

Key Responsibilities

Reviews Plan of Care prior to making each visit to ensure the correct care is given to every patient assigned.

• Provides or assists with personal care, including but not limited to bathing, dressing, oral hygiene, hair care, meal preparation, feeding, transfers and exercises based on plan of care.
• Maintains comfortable clean environment by adjusting lighting, room temperature, changing bed linens and light housekeeping as needed. Alert to safety factors and accident prevention.
• Reports results timely and accurately to the RN for any vital signs, or other equipment readings as ordered by the plan of care.
• Participates as a member of the interdisciplinary team and communicates with the RN any patient changes or updates to ensure proper response time and quality patient care.
• Documents activities carried out into the electronic health record at the point of care.
• Follows individualized plan of care, documenting comprehensive observations, interventions and all communication with supervising RN.
• Recognizes and reports changes in mental or physical status of patient promptly to RN to provide sufficient communication to team members.
• Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio’s Hospice patients.
• Participates in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services.
• Precepts new staff to support professional relationships with newly hired team members as required.
