Job summary

YAI - Seeing beyond disability in New Jersey seeks a dedicated Direct Support Professional to assist individuals with intellectual and developmental disabilities in daily living, social activities, and community integration.

You will provide personal care, monitor health and safety, implement Behavior Support Plans, document progress, and collaborate with a multidisciplinary team to promote independence and a high quality of life.

Qualifications

• Must be at least 18 years old with HS diploma or equivalent.
• Valid driver’s license with no more than five points; ability to perform physical tasks including lifting up to 25 pounds.
• Strong written and oral communication; ability to document services for billing and tracking.

Responsibilities

• Assist individuals to maximize strengths and quality of life.
• Provide personal care and assist with ADLs per individualized plans.
• Monitor behavior and implement BSPs/ISPs and safety protocols.
• Transport individuals to activities and medical appointments.
• Maintain documentation and communicate with clinical team.
