Qualifications

Responsibilities

• Participate in residents' daily activities to foster a person-centered atmosphere.
• Assist in planning and implementing the program and goals.
• Support advocacy and independent living skills, including health plans and community integration.
• Follow health care plans, use medical equipment, and respond to emergencies.
• Ensure safety and provide personal care as outlined in habilitation plans.
• Maintain safety and report incidents per policy.
