Qualifications

• Must have high school diploma or equivalent.
• Experience in direct support or caregiving is preferred.
• Ability to read, write, and follow a care plan.
• Effective verbal, written, and interpersonal skills.
• Basic to intermediate computer skills.
• Valid driver’s license and reliable transportation preferred.

Responsibilities

• Interact with individuals regularly and address concerns.
• Connect individuals to community activities to support health.
• Provide encouragement and coaching for daily living skills.
• Assist with shopping for personal needs and desires.
• Coach on sound financial management decisions.
• Assist with meal planning, shopping, and food preparation.
• Perform routine motion and ambulation assistance.
• Administer medications as directed and report changes.
• Document care and submit records for client files.
• Maintain infection control and safe practices.
• Communicate with interdisciplinary team during meetings.
