Job summary

Southeastern Integrated Care in Lumberton, NC seeks a Qualified Professional - ACT to provide direct and indirect interventions under supervision and contribute to the Person-Centered Plan.

The role emphasizes documentation, confidentiality, and coordination with Team Leaders for discharge planning and community resource linkage. Relevant MH/DD/SAS experience is preferred; a Human Services degree or RN license is required.

Qualifications

• Bachelor’s degree in Human Service field with 2 years MH/DD/SAS experience, OR
• Bachelor’s degree in non-Human Services with 4 years MH/DD/SAS experience, OR
• RN with 4 years MH/DD/SAS experience, OR
• Master’s in Human Services with 1 year MH/DD/SAS experience

Responsibilities

• Develop and update the initial and ongoing Person-Centered Plan (PCP).
• Advocate for involvement of the person and family in treatment and progress monitoring.
• Coordinate discharge planning based on PCP goals.
• Complete intake packets and maintain timely, accurate documentation.
• Maintain confidentiality and protect sensitive information.
• Coordinate referrals and linkage to community resources.
• Submit all documentation accurately and on time.
