Qualifications

• Bachelor's degree in Human Service field with 2 years full-time MH/DD/SAS experience with population.
• Bachelor's degree in a field other than Human Services with 4 years full-time MH/DD/SAS experience with population.
• An RN in North Carolina with 4 years MH/DD/SAS experience with population.
• Master's degree in Human Services with 1 year full-time MH/DD/SAS experience.

Responsibilities

• Develop and update the initial and ongoing Person-Centered Plan.
• Advocate for client and family participation in treatment and progress monitoring.
• Serve as support staff under Team Lead.
• Coordinate PCP activities under Team Leader.
• Link and refer services; schedule community resources.
• Document billable encounters in the EMR within 24 hours.
• Maintain confidentiality and privacy of protected information.
