Job summary

Stark County Board of Developmental Disabilities in Canton, Ohio seeks an SSA to coordinate services for eligible individuals and develop the ISP to ensure people live their best lives.

You will monitor services, maintain budgets, and collaborate with families, guardians, and providers in person-centered planning. A valid Ohio driver’s license and a related bachelor’s degree are required, with DODD Certification eligibility.

Qualifications

• Bachelor’s Degree in related field is required.
• Eligible for DODD Certification or pursuing certification.
• Must hold a valid Ohio driver’s license and proof of insurance.

Responsibilities

• Coordinate services and supports for individuals onISP as part of Person-Centered Planning.
• Develop and monitor the Individual Service Plan (ISP) and authorize services within budget.
• Provide follow-up discussions via various contacts to achieve outcomes and health/welfare goals.
• Complete TCM notes daily; ensure Medicaid billing and compliance with state rules and board policies.
• Assist individuals to receive services with least restrictive options and connect to community resources.
• Maintain excellent customer service to support the Board’s vision and mission.
