Job summary

Lutheran Social Service of Minnesota is seeking a Permanent Supportive Housing Case Manager to provide case management services and housing intervention for families and individuals experiencing homelessness. You will assess needs, coordinate services, and support stable housing outcomes.

The role requires a Bachelor's degree in Human Services or equivalent, a valid driver's license, and at least two years in human services.

Qualifications

• Bachelor's degree in Human Services or equivalent experience required.
• Valid driver's license.
• At least two years of experience in providing direct or indirect social services.
• Knowledge of counseling theory and techniques - chemical dependency treatment and physical or sexual abuse treatment is required.
• Knowledge of Harm Reduction and Housing First Model.
• Ability to work independently.
• Ability to work with individuals from diverse backgrounds.
• Strong organizational skills.
• Demonstrated ability to communicate effectively, positively, and professionally with participants, providers, family members, and others.
• Able to promote a positive, supportive, and collaborative atmosphere.
• Previous experience working with low-income families and those experiencing homelessness preferred.
• Social Work license preferred.
• Training in working with low-income families and those who are homeless preferred.
• Knowledge of related issues in chemical dependency, mental health, parenting, sex trafficking, and domestic violence preferred.
