• The ability to assess, plan, deliver and document care within scope and to complete applicable client screening and credential requirements.

Responsibilities

• Assess communication, cognitive-linguistic and swallowing needs within your professional scope.
• Develop individualized goals and treatment plans, deliver appropriate interventions, monitor progress and adapt care as needs change.
• Collaborate with the rehabilitation and care team so treatment goals connect with each person’s daily activities.
• Document evaluations, interventions and progress accurately and promptly, communicate relevant changes, and follow professional standards, safety practices and applicable requirements.
