Job summary

Relient Health is seeking a licensed Speech Language Pathologist (SLP) for a full-time role at a Skilled Nursing Facility in Hayward, CA. You will evaluate and treat patients with communication and swallowing disorders, and collaborate with a multidisciplinary team.

Qualifications

• Must have graduated from an accredited Speech-Language Pathology program.
• CA SLP license or eligibility required; CCC-SLP preferred, CFs considered with supervision.

Responsibilities

• Evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders.
• Develop and implement individualized treatment plans.
• Document patient progress using EMR systems.
• Collaborate with PTs, OTs, nurses, and physicians to deliver coordinated care.
• Educate patients, families, and staff on treatment goals and progress.
