Talented Medical Solutions is seeking a Speech-Language Pathologist (SLP) for a permanent position in Fremont, California. The role offers a Monday–Friday, 35-hour work week with no weekend rotation, at a 126-bed skilled nursing facility.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current California Speech-Language Pathologist license or eligibility for California licensure.
• Certificate of Clinical Competence (CCC) from ASHA or eligibility preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• New graduates are encouraged to apply.

Responsabilités

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on residents' clinical needs and goals.
• Provide skilled speech-language therapy using evidence-based practices.
• Monitor patient progress and adjust treatment plans as appropriate.
• Evaluate and treat communication, cognition, and swallowing disorders.
• Complete accurate and timely clinical documentation.
• Participate in interdisciplinary care planning and collaborate with nursing and other rehabilitation professionals.
• Educate residents, families, and caregivers on treatment strategies and recommendations.
• Communicate effectively with the care team regarding patient progress and changes in condition.
• Promote patient safety, independence, and quality of life through individualized care.
