Talented Medical Solutions seeks a Mobile Outpatient Physical Therapist (PT) in Holyoke, MA. You will evaluate patients, deliver PT in a mobile setting, and create individualized treatment plans across diverse patient needs.

Evaluate patients and develop individualized outpatient treatment plans
Deliver skilled physical therapy services in a mobile setting
Provide therapeutic exercises, functional training, manual therapy, and other appropriate interventions
Monitor patient progress and modify treatment plans based on clinical response
Educate patients and caregivers on exercises, mobility, home programs, and injury prevention
Maintain accurate and timely clinical documentation
Communicate effectively with patients, families, and other healthcare professionals
Manage travel between patient locations within the designated service area
Doctor of Physical Therapy (DPT) or equivalent degree
Current Massachusetts Physical Therapist license
Strong clinical judgment and communication skills
Comfortable providing care in a mobile outpatient environment
Ability to manage time effectively and work independently
Reliable transportation and willingness to travel between patient locations
