Complete physical therapy evaluations and assessments
Develop individualized treatment plans based on patient needs and goals
Provide skilled therapeutic interventions
Help patients improve mobility, strength, balance, endurance, and functional independence
Monitor patient response to treatment and adjust plans as needed
Document evaluations, treatments, progress, and outcomes accurately
Educate patients and caregivers regarding therapy and safe mobility
Collaborate with nursing, therapy, physicians, and other members of the care team
Promote patient safety and quality outcomes
Doctor of Physical Therapy (DPT) or equivalent degree
Current Massachusetts Physical Therapist license
Strong communication and clinical skills
Patient-centered approach to care
Ability to work effectively in an SNF environment
