Qualifications

  • Licensed Physical Therapist in Massachusetts.
  • Degree from an accredited Physical Therapy program.
  • CPR certification (current).
  • Valid driver's license and reliable transportation.
  • Home health experience required.
  • Strong communication, organization, and clinical decision-making skills.

Responsibilities

  • Provide in-home physical therapy services, including evaluations, start of care visits, follow-ups, recertifications, and discharges.
  • Develop and implement personalized treatment plans based on patient needs and physician orders.
  • Guide patients through therapeutic exercises, gait training, balance and mobility activities.
  • Educate patients and caregivers on safety, fall prevention, and daily living strategies.
  • Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
  • Assess vital signs as needed and communicate any concerns to the care team.
  • Recommend and educate on the use of assistive devices and DME.
  • Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians.
  • Complete timely and accurate documentation.
  • Support discharge planning and ensure continuity of care.

What You'll Do

  • Develop and implement personalized treatment plans based on patient needs and physician orders
  • Guide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvement
  • Educate patients and caregivers on safety, fall prevention, and daily living strategies
  • Monitor patient progress and adjust treatment plans to achieve optimal outcomes
  • Assess vital signs as needed and communicate any concerns to the care team
  • Recommend and educate on the use of assistive devices and durable medical equipment (DME)
  • Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians
  • Complete timely and accurate documentation
  • Support discharge planning and ensure continuity of care
What We're Looking For
  • Licensed Physical Therapist in the state of Massachusetts
  • Degree from an accredited Physical Therapy program
  • CPR certification (current)
  • Valid driver's license and reliable transportation
  • Home health experience required
  • Strong communication, organization, and clinical decision-making skills
Preferred
  • At least 1 year of clinical experience

Work Environment & Physical Requirements
