Job summary

Texas-Family-Care-Network is seeking a Courtesy Case Manager to ensure child welfare when children are removed from home. You will work with parents, extended family, and community partners to identify resources and plan for permanency in a child-centered, culturally competent, and strengths-based manner.

The role involves supervision and service delivery, coordination with education/health/justice systems, and court involvement to improve outcomes.

Qualifications

• Education options include a Bachelor’s degree or an equivalent combination of education and experience.
• Valid state driver’s license and safe driving record are required.
• Reliable transportation and current auto insurance are required.
• Successful completion of the case manager 13-week training and certification.

Responsibilities

• Monitors child/youth placement needs and collaborates with the Placement staff as needed.
• Coordinates regular contact between the child and his/her biological family as appropriate.
• Participates in meetings and conferences at times and places convenient for the family and others involved.
• Lears background of the family and child/youth to strategize how best to support them.
• Uses tools and training to determine needs and makes appropriate referrals for testing, evaluations, records, or other assessments.
• Works with other systems (education, health, and justice) to improve outcomes for the child/youth and family.
• Documents case records in accordance with policy and records data timely in CoBris/IMPACT.
• Attends court hearings about the child and family and adheres to safety policies.

Education

Bachelor’s degree from an accredited college or university

Associate's degree from an accredited college or university plus two years’ relevant work experience

60 accredited college credit hours plus two years’ relevant work experience

90 accredited college credit hours plus one year of relevant work experience
