The position emphasizes documentation in the EMR, case conferencing with the care team, and ongoing collaboration to address unmet patient needs across the care continuum.

Qualifications

• High School Diploma or GED.
• 1-2 years' experience completing needs assessments. Familiarity with patient resources in the local surrounding area.
• Mental Health First Aid within 6 months of hire.
• Some college preferred.

Responsibilities

• Coordinate patient care across a network of providers and services.
• Maintain EMR documentation and participate in care team case conferences.
• Engage hard-to-reach patients to improve health outcomes.
• Participate in needs assessment and networking activities and in ED testing and counseling as assigned.
• Track referrals and follow up to address unmet patient needs.
