Job summary

Chapa-De Indian Health in Grass Valley, CA is seeking a Perinatal Nurse Case Manager to empower expectant families with compassionate, continuous care during pregnancy.

You will conduct OB pre-visit intake, coordinate between OB providers and Labor and Delivery, and manage follow-up and referrals for antenatal testing. Active California RN license, OB/NST experience, and 2 years of clinical/education background are required. Hourly pay $52.65–$60.00.

Qualifications

• Current and good standing RN License issued by the State of California.
• Two years of clinical experience or experience in health education.
• One year of OB experience and/or NST experience.

Responsibilities

• Conduct pre-visit OB intake appointments with all new OB patients.
• Serve as the main point of contact and coordinator between Chapa-De OB providers, Camellia Women's Health and delivering Labor and Delivery Department.
• Manage follow-up care and referrals for antenatal testing of OB patients as directed by the OB providers.
