Job summary

Care New England seeks a care manager to support a Collaborative Care team in prenatal clinics. You will coordinate mental health services during pregnancy and up to one year postpartum, screen and educate patients, and facilitate referrals to external services as needed.

You will collaborate with obstetric providers and mental health clinicians, monitor symptoms, support medication management, and track outcomes in a registry to ensure continuity of care.

Qualifications

• Experience in perinatal mental health care settings.
• Experience screening, assessment and treatment planning for mental health and substance use.
• Familiarity with evidence-based psychosocial treatments and brief interventions.
• Ability to collaborate effectively in a multidisciplinary team.
• Strong written and verbal communication skills.

Responsibilities

• Collaborate with obstetric providers and mental health clinicians to support mental health care.
• Screen and assess patients for mental health and substance use disorders.
• Educate patients about mental health conditions and treatment options.
• Monitor patients for changes in symptoms and treatment side effects.
• Support medication management and adherence in collaboration with clinicians.
• Provide psychoeducation on medications during pregnancy and postpartum.
• Deliver brief interventions using evidence-based techniques.
• Facilitate referrals to psychotherapy when needed.
• Participate in weekly caseload consultations and communicate recommendations to the care team.
• Encourage patient engagement and follow-up in care.
• Track follow-up and outcomes using a registry and document encounters.
• Document progress and treatment recommendations in the registry.
• Adjust treatment plans in consultation with the care team as needed.
• Facilitate referrals to services outside the prenatal clinic.
• Develop relapse prevention plans for patients in remission.
