Healthy Start Inc. in Pittsburgh, PA, seeks an experienced Community Health Worker to provide field-based case management, coordination of care, and health education for program participants. This role is the first line of care in our model and focuses on maternal and child health.

You will enroll eligible participants, manage up to 48 cases, and develop plans of care. Education covers breastfeeding, immunization, safe sleep, family planning, and access to resources.

Qualifications

• High school diploma or GED and three or more years of experience in a social service/health field.
• Bachelor's or advanced degree preferred; combinations of education and experience considered.

Responsabilités

• Identify and enroll eligible participants.
• Provide home- and community-based case management and coordination services.
• Maintain a full caseload of up to 48 participants.
• Assist in developing and managing the participant’s plan of care/goal planning.
• Provide education on breastfeeding, immunization, safe sleep, family planning, smoking cessation, nutrition, mental health, physical health, stress, and domestic violence.
• Screen for depression and refer to treatment as necessary.
• Document all contacts and services provided.
• Connect participants to housing, transportation, food and mental health resources.
• Promote internal referral programs and conduct service plan meetings.
• Identify gaps in services and devise remedies in health-related areas.
• Present programs on health information to community groups.
• Complete trainings related to the Healthy Start model and public health.
• Attend meetings, events, and conferences as necessary.
