Responsibilities

• Serve as the primary navigation contact for pregnant, postpartum, maternal, infant, and family populations.
• Coordinate scheduling of prenatal, postpartum, pediatric, and other appointments; support continuity of care.
• Assist with identifying barriers to care and coordinate with team to resolve them.
• Provide patient education and health promotion related to maternal and infant health.
