Job summary

Northeast Community Clinic is seeking a Medical Assistant-level perinatal health worker in Los Angeles, CA to support CPSP services. You will orient patients to perinatal care, perform comprehensive assessments across obstetric, nutrition, education and psychosocial domains, and create/update the ICP with patient input.

The role includes education, referrals, and postpartum care coordination. The position requires at least a Medical Assistant certification or higher, 1+ year of perinatal

Qualifications

• Microsoft Office (Word, Excel, Outlook)
• Excellent communication and customer service skills
• Bilingual English/Spanish preferred; 10% travel required
• CPHW certification and clinical perinatal experience required
• CPR/BLS certification and valid ID required

Responsibilities

• Orient the patient to comprehensive perinatal care at entry of care and throughout the pregnancy as needed
• Complete assessments in obstetric, nutrition, health education and psychosocial using approved forms
• Develop the Individualized Care Plan (ICP) with the patient and revise as needed
• Provide Perinatal Education, counseling, referrals and instructions as identified on the ICP
• Reassess each trimester to identify changes and provide continued support
• Postpartum assessment and care plan for mother and infant
• Track attendance at appointments and identify reasons for missed visits
• Assist patient to obtain Medi-Cal and explain required documents
• Maintain logs and monthly reports to program leadership
