Job summary

Pomelo Care is seeking a registered nurse to complete initial intake and crisis consultation to support perinatal behavioral health care coordination. This remote role reports to the Behavioral Health Lead and is open to candidates in the continental US.

Responsibilities include risk assessments, care plan development, psychoeducation for patients and families, progress monitoring, and advocating for patient autonomy and dignity while collaborating with obstetricians, nurses, care coordinators,

Qualifications

• RN with unrestricted multi-state compact license.
• 4+ years experience in hospital/healthcare practice serving maternal-child health populations.
• Experience or advanced training in psychiatric nursing, perinatal mental health or related field.
• Experience working with pregnant and postpartum individuals and their families.

Responsibilities

• Conduct initial patient risk assessments and triage based on urgency and severity of mental health conditions.
• Collaborate with a multidisciplinary team of healthcare professionals, including obstetricians, APPs, nurses, care coordinators and therapists.
• Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
• Provide psychoeducation to patients and their families about perinatal mental health conditions and treatment options.
• Monitor patients' progress and adjust care plans as necessary.
• Advocate for patients' rights and ensure care respects autonomy and dignity.
• Participate in ongoing professional development and training to stay up-to-date with best practices in perinatal mental health nursing.
