Job summary

Pomelo Care is the leading virtual medical practice for women and children, delivering 24/7 clinical care across pregnancy, postpartum, pediatrics, and menopause.

This remote role coordinates perinatal behavioral health care, reporting to the Behavioral Health Lead, with responsibilities including risk assessments, team collaboration, care plan development, psychoeducation, and progress monitoring.

Qualifications

• Registered Nurse with an unrestricted multistate compact license.
• 4+ years experience in hospital/healthcare practice serving maternal-child health populations.
• Experience or advanced training in psychiatric nursing, perinatal mental health or related field.
• Experience working with pregnant and postpartum individuals and their families.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced, metrics-driven environment.
• Commitment to patient-centered care and holistic mental health.

Responsibilities

• Conduct initial patient risk assessments and triage based on urgency and severity of mental health conditions.
• Collaborate with a multidisciplinary team including obstetricians, APPs, nurses, care coordinators and therapists.
• Develop and implement individualized care plans for patients.
• Provide psychoeducation to patients and families about perinatal mental health and treatment options.
• Monitor patients' progress and adjust care plans as necessary.
• Advocate for patients' rights and ensure care respects autonomy and dignity.
• Participate in ongoing professional development and training.
