Job summary

Evolving Solution Services is seeking an experienced Healthcare (ACO) Performance Improvement Project Manager to lead strategic initiatives that boost ACO performance, enhance care quality, and reduce costs while adhering to MSSP and regulatory requirements.

The ideal candidate combines strategic thinking with strong analytical and project management skills, collaborates across clinical and operational teams, and communicates findings clearly to executives and committees.

Qualifications

  • Bachelor’s degree required; Master’s degree preferred.
  • Minimum of 5 years of experience in healthcare project management, quality improvement, or performance improvement.
  • Experience within an ACO or value-based care setting preferred.

Responsibilities

  • Lead cross-functional projects to improve ACO performance metrics.
  • Develop and manage detailed project plans, timelines, and deliverables.
  • Analyze performance data to identify gaps and opportunities for improvement.
  • Implement evidence-based interventions to address identified gaps.
  • Collaborate with clinical and operational leaders to align initiatives.
  • Facilitate communication among providers, payers, and stakeholders.
  • Ensure all projects comply with MSSP and regulatory requirements.
  • Monitor updates to policies and adjust initiatives as necessary.
  • Monitor KPIs and prepare performance reports for leadership and committees.
  • Develop training programs to support staff and providers.

Job description

The Healthcare (ACO) Performance Improvement Project Manager will lead and support strategic initiatives to enhance the performance of the Accountable Care Organization (ACO). This role focuses on operational efficiency, quality improvement, and cost reduction while ensuring alignment with regulatory requirements, organizational goals, and patient-centered care principles.

The ideal candidate is a strategic thinker with strong analytical and project management skills, capable of collaborating across multidisciplinary teams to drive measurable improvements.

Key Responsibilities:

  • Project Management: Lead cross-functional projects aimed at improving ACO performance metrics, including quality of care, patient outcomes, and cost efficiency.
  • Project Management: Develop and manage detailed project plans, timelines, and deliverables.
  • Performance Improvement: Analyze performance data to identify gaps and opportunities for improvement.
  • Performance Improvement: Implement evidence-based interventions and best practices to address identified gaps.
  • Stakeholder Collaboration: Partner with clinical and operational leaders to ensure alignment and integration of performance improvement initiatives.
  • Stakeholder Collaboration: Facilitate communication and engagement among providers, payers, and other stakeholders.
  • Regulatory Compliance: Ensure all projects comply with federal and state regulations, including Medicare Shared Savings Program (MSSP) requirements.
  • Regulatory Compliance: Monitor updates to ACO policies and adjust initiatives as necessary.
  • Data Analysis and Reporting: Leverage data analytics to monitor key performance indicators (KPIs) and measure project outcomes.
  • Data Analysis and Reporting: Prepare and present performance reports to executive leadership and ACO committees.
  • Education and Training: Develop training programs to support staff and provider understanding of performance improvement initiatives.

  • Ability to work independently and manage competing priorities in a fast‑paced environment.

Certifications:

  • Lean Six Sigma or other performance improvement certifications preferred.

Education and/or Experience:

· Bachelor’s degree in healthcare administration, public health, business, or a related field.

· Master’s degree preferred.

· Minimum of 5 years of experience in healthcare project management, quality improvement, or performance improvement, preferably within an ACO or value-based care setting.

· Proven track record of managing multiple projects and driving measurable results.

Ability to read and comprehend simple instructions, correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one‑on‑one situations to visitors, clients, and other employees of the organization.

Reasoning Ability:

Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
