Qualifications

  • Bachelor’s degree in healthcare administration, public health, business, or related field.
  • Master’s degree preferred.
  • Minimum of 5 years of healthcare project management, quality improvement, or performance improvement experience.
  • Proven track record of managing multiple projects and driving measurable results.

Responsibilities

  • Lead cross-functional projects to improve ACO performance metrics (quality of care, outcomes, cost).
  • Develop and manage detailed project plans, timelines, and deliverables.
  • Analyze performance data to identify gaps and opportunities for improvement.
  • Partner with clinical and operational leaders to ensure alignment and integration of initiatives.
  • Ensure projects comply with MSSP requirements and regulatory updates.
  • Leverage data analytics to monitor KPIs and measure outcomes.
  • Prepare and present performance reports to executive leadership and committees.
  • Develop training programs to support staff and providers.
  • Promote a culture of continuous improvement.

Job description

Key Responsibilities:
  • Project Management:
  • Lead cross-functional projects aimed at improving ACO performance metrics, including quality of care, patient outcomes, and cost efficiency.
  • Develop and manage detailed project plans, timelines, and deliverables.
  • Performance Improvement:
  • Analyze performance data to identify gaps and opportunities for improvement.
  • Implement evidence-based interventions and best practices to address identified gaps.
  • Stakeholder Collaboration:
  • Partner with clinical and operational leaders to ensure alignment and integration of performance improvement initiatives.
  • Facilitate communication and engagement among providers, payers, and other stakeholders.
  • Regulatory Compliance:
  • Ensure all projects comply with federal and state regulations, including Medicare Shared Savings Program (MSSP) requirements.
  • Monitor updates to ACO policies and adjust initiatives as necessary.
  • Data Analysis and Reporting:
  • Leverage data analytics to monitor key performance indicators (KPIs) and measure project outcomes.
  • Prepare and present performance reports to executive leadership and ACO committees.
  • Education and Training:
  • Develop training programs to support staff and provider understanding of performance improvement initiatives.
  • Promote a culture of continuous improvement across the organization.

Skills and Competencies:
  • Strong understanding of ACO models, value-based care, and healthcare quality metrics.
  • Proficiency in project management tools and methodologies (e.g., PMP certification preferred).
  • Exceptional analytical skills with experience in data-driven decision-making.
  • Excellent communication and interpersonal skills, with the ability to engage and influence stakeholders.
  • Ability to work independently and manage competing priorities in a fast-paced environment.
Certifications:
  • Lean Six Sigma or other performance improvement certifications preferred.
Education and/or Experience:

Bachelor’s degree in healthcare administration, public health, business, or a related field.

Master’s degree preferred.

Minimum of 5 years of experience in healthcare project management, quality improvement, or performance improvement, preferably within an ACO or value-based care setting.

Proven track record of managing multiple projects and driving measurable results.

Ability to read and comprehend simple instructions, correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one situations to visitors, clients, and other employees of the organization.

Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
