Summary
The Junior Project Manager – ACO REACH Program Operations supports the execution and day-to-day management of ACO REACH initiatives under the direction of the ACO REACH Project Manager. This role assists in coordinating timelines, deliverables, and reporting across multiple departments to ensure operational excellence and compliance with program requirements.
The Junior Project Manager contributes to project planning, documentation, data validation, and communication efforts to drive successful program implementation and measurable performance improvement. This position is ideal for an early-career professional seeking to develop project management skills in value-based care and ACO operations.

Essential Duties And Responsibilities
Program & Operational Support
  • Support the ACO REACH Project Manager in coordinating all program activities, meetings, and reporting deadlines.
  • Assist in translating leadership direction into actionable project tasks, timelines, and deliverables.
  • Track progress toward milestones, flag risks or delays, and help maintain accurate project documentation.
  • Coordinate across departments (Finance, Data, Quality, Network Development, Performance Improvement) to support timely data sharing, reporting, and payment processes.
  • Help prepare agendas, notes, and follow-ups for internal and external meetings related to REACH program operations.
Financial and Data Support
  • Assist with monthly participant payment file validation and reconciliation under supervision of the REACH Project Manager.
  • Support the analysis of payment data and performance metrics to identify trends or discrepancies.
  • Maintain organized records of participant communications, payment documentation, and audit materials.
Stakeholder Engagement
  • Help respond to participant inquiries and elevate issues to the ACO REACH Project Manager as needed.
  • Contribute to participant education efforts, presentations, and onboarding materials.
  • Support communication between internal teams and external stakeholders to ensure consistent follow-through.
Reporting & Compliance
  • Support preparation of required reports for CMMI, internal leadership, and participants.
  • Assist in maintaining program documentation to ensure compliance with CMS and organizational policies.
  • Help track and update key performance indicators (KPIs) related to REACH operations.
Project Management & Process Improvement
  • Contribute to the creation and maintenance of project plans, timelines, and task trackers.
  • Participate in process improvement initiatives aimed at streamlining workflows and improving accuracy.
  • Assist with documentation of lessons learned and help implement improvements for future project cycles.
Travel Requirement
  • Travel required up to two times per month, including overnight stays, to meet with participants, attend events, or conduct site visits. Frequency may vary based on company needs.
Key Performance Indicators (KPIs)
  • Timeliness: 95% of assigned deliverables completed by established deadlines.
  • Accuracy: ≥ 98% accuracy on validated reports, trackers, and payment support documents.
  • Communication: 100% of assigned meeting notes distributed within 48 hours.
  • Collaboration: Positive feedback from internal teams and project manager on reliability and initiative.
  • Compliance: Zero compliance or documentation issues in assigned tasks.

  • Technical Skills: Proficient in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook). Familiarity with project management software or data visualization tools (e.g., Power BI, Smartsheet, Asana) a plus.
  • Knowledge Base: Basic understanding of value-based care, CMS/CMMI programs, or ACO structures preferred.
Physical Demands
