Job summary

Day Kimball Health in Thompson, CT seeks a Per Visit Registered Nurse (RN) to deliver home-based nursing care. The RN provides comprehensive nursing services to individuals and families, acting as a primary care nurse on an interim basis.

The role requires carrying out nursing duties with attention to patient confidentiality, documenting care on portable devices or paper records, and collaborating with the Clinical Supervisor to ensure high-quality, patient-centered care at home.

Responsibilities

• Provides skilled nursing care to patients in their homes (including admission/discharge) on a family centered basis while recording/maintaining complete patient record on Portable Electronic Device and/or paper record.
• Teaches, demonstrates, supervises and evaluates care given by peers, HHAs, family members and significant others.
• Performs venipuncture to administer IV drip medication, TPN or lipids, which have been approved for home administration.
