Job summary
Interim HealthCare is seeking a Home Health Registered Nurse (RN) for per diem assignments in the CT area. You will decide how much you work and how your days are structured, delivering compassionate home-based care and collaborating with a patient-centered team.
You will work with physicians, oversee LPNs/CNAs, educate patients and families, document progress, and coordinate discharge. Per visit pay, 1:1 nurse-to-patient ratios, flexible assignments, and online CEUs are offered.
Qualifications
- Graduate of an accredited nursing program with active RN license in CT.
- Minimum of 1 year of nursing experience, ideally in home healthcare.
- Knowledge of state and federal home health regulations.
- Strong communication skills and compassion.
Responsibilities
- Provide home-based care to patients after discharge from hospital or facility.
- Work with physicians and oversee LPNs, CNAs and aides providing patient care.
- Examine patients, perform assessments, document progress, and report changes.
- Educate patients on plan of care, medications, therapy, diet and exercise.
- Coach family members on the patient’s plan of care, medication and home safety.
- Coordinate discharge from services and ensure goals are met.
