Job summary

Visiting Nurse Association in Massachusetts is seeking a Registered Nurse per visit to provide comprehensive home health care services, including assessment, plan of care, and coordination with the care team.

You will perform admissions assessments, develop and revise plans, coordinate discharges, and educate patients and caregivers. A MA RN license, CPR, and home health/IV experience are required or BSN preferred.

Qualifications

• Current RN license in Massachusetts.
• Minimum one year of nursing experience or 6 months with a BSN.
• CPR certification required.
• Home care and IV experience, computer skills and BSN preferred.

Responsibilities

• Perform a comprehensive nursing assessment upon admission, including physiological, psychosocial, environmental factors and health history.
• Develop, implement and revise the patient’s plan of care; coordinate with the interdisciplinary team.
• Initiate and document discharge planning and related paperwork.
• Educate patient and caregiver; ensure documentation is clear and timely.
• Notify involved care providers of significant changes in the patient’s condition.
• Maintain records of narrative documentation reflecting skilled care.
