Job summary

Paycom - ATS is seeking a compassionate Registered Nurse to join our home health team in New Bedford, MA. The role involves visiting patients at home, coordinating care with physicians and other professionals, and ensuring timely documentation and eligibility reviews.

Ideal candidates hold an MA RN license, have at least one year of home care experience, and are fluent in English. The position operates on a pay-per-visit model with standard home health responsibilities and patient-centered care

Qualifications

• Graduate of an approved professional nursing program.
• Currently licensed in MA in good standing.
• Minimum of one year experience as a professional nurse, preferably in home care.
• Must understand and communicate fluently in English.

Responsibilities

• Demonstrates proficiency with nursing skills in preventative and rehab nursing procedures.
• Coordinates total plan of care with other healthcare professionals involved in care and helps to achieve continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
• Makes the initial and/or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within state-specific guidelines, and submits accurate documentation within 24 hours.
