Job summary

capecodhc is seeking a qualified Registered Nurse to perform home health visits in Massachusetts, assessing patient needs, initiating and revising plans of care, and coordinating with the interdisciplinary team.

The role requires current RN license in MA, at least 1 year of nursing experience (or 6 months with a BSN), CPR certification, and ability to document clearly. Pre-hire medication test and IV/home care experience preferred.

Qualifications

• Current RN license in Massachusetts (Massachusetts).
• Minimum one year of nursing experience or 6 months with a BSN.
• CPR certification required.
• Home health and IV care experience preferred.
• Successful passage of VNA pre-hire medication test.

Responsibilities

• Assess patient needs and implement revisions to the care plan.
• Coordinate interdisciplinary case conferences and document outcomes.
• Instruct and supervise Home Health Aide care every 14 days.
• Notify physician and care team of significant changes in patient condition.
• Complete admission and discharge paperwork in a timely manner.
• Document narrative care recordings including teaching and patient responses.
