Job summary

Jethro Health System LLC in Lynn, MA is seeking a dedicated Registered Nurse (RN) to provide skilled home health visits. This per-diem, part-time role offers flexibility and one-on-one patient care in patients' homes, with driving to visits required.

The ideal candidate holds an active MA RN license with at least 1 year of RN experience, preferably in home health. You will conduct assessments, administer treatments, document care, and collaborate with physicians and the care team.

Qualifications

• Active Massachusetts RN license in good standing.
• Minimum 1 year of RN experience (home health experience preferred).
• Strong assessment, documentation, and communication skills.
• Ability to work independently and manage time efficiently.
• Valid driver''s license, reliable transportation, and willingness to travel.
• Comfortable using EMR and mobile documentation systems.

Responsibilities

• Provide skilled nursing care in patients' homes per physician orders and care plans.
• Perform comprehensive assessments, vital signs, and skilled nursing interventions.
• Administer medications, wound care, injections, and treatments.
• Educate patients and caregivers on medications, disease processes, and safety.
• Monitor patient progress and report changes in condition.
• Complete accurate, timely documentation for each visit.
• Communicate with physicians, case managers, and interdisciplinary team members.
• Follow all home health regulations and agency policies.
