Responsibilities

• Assists patients to reach maximum performance and level of functioning.
• Evaluates, sets goals, plans, and executes treatment based on medical referral; reports findings to health care team.
• Provides discharge recommendations, exercise programs, and family teaching.
• Determines need to refer patients to other diagnostic, medical, and therapeutic services.
• Documents and records timely; coordinates scheduling of patients and meetings.
• Maintains department safety and equipment operation.
