Job summary

CenterWell Home Health is seeking a per diem Speech Language Pathologist to deliver in-home SLP services. You will evaluate patients, develop and periodically review care plans, and implement therapy to improve speech, language, voice, swallowing and cognitive communication.

You will document progress, coordinate with the care team, and educate patients and families in home settings. The role requires ASHA CCC or licensure, a minimum of six months experience, current CPR, a valid license, and

Qualifications

• Certification of Clinical Competence in Speech Language Pathology (ASHA) or Audiology
• Minimum six months experience as a speech therapist/speech-language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects
• Administer and interpret diagnostic tests and applications of therapeutic treatments
• Observe, record and report changes in the patient's condition and response to treatment
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation as required by the Company and/or facility; prepare reports
• Participate in care coordination activities and discharge planning
• Maintain confidentiality of patient information
