Mission Healthcare in Modesto, CA, seeks an experienced Speech-Language Pathologist to evaluate patients in home-health settings and build tailored plans of care. You will provide therapy for speech, language, voice, and swallowing disorders, implement personalized treatment approaches, and coordinate with physicians and the clinical team.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• CCC-SLP certification from ASHA.
• State license to practice as a Speech-Language Pathologist.
• Minimum of one year of clinical SLP experience; home health preferred.
• Current BLS (CPR) certification; hands-on training required.
• Excellent communication, documentation, and organizational skills.

Responsibilities

• Conduct thorough speech-language evaluations in the home environment.
• Develop and implement personalized plans of care for speech, language, voice, and swallowing disorders.
• Provide therapy using specialized techniques and equipment.
• Coordinate care with referring physicians and the clinical team.
• Document evaluations, treatment plans, and progress.
• Supervise home health aides when related to the plan of care.
• Educate patients, caregivers, and families on therapy programs and goals.
• Participate in case conferences and interdisciplinary meetings.
• Maintain up-to-date knowledge of SLP practices.
• Identify patient needs and make appropriate referrals.
