Job summary

Creative Solutions In HealthCare is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house rehab team serving residents in a SNF. You will provide skilled evaluations, develop treatment plans, and support communication and swallowing goals with person-centered care.

Texas licensure and current ASHA certification are required, with good standing among licensing boards. New grads are welcome; flexible schedules and competitive compensation are offered.

Qualifications

Experience in SNF/long-term care settings.
Active Texas SLP license and ASHA certification.
Good standing with regulatory boards.

Responsibilities

Evaluate residents for swallowing or cognitive deficits.
Develop and implement individualized treatment plans.
Provide AAC systems for language disorders.
Educate residents, families and caregivers.
Coordinate with other professionals for swallowing procedures.
Document billing and clinical notes timely.
Maintain resident confidentiality and comply with laws.

Net Health documentation

Patient management flexibility

Education

Active Texas license in Speech-Language Pathology
