Responsabilités

• Provide Social Work coverage to facilitate discharges and meet with patients and families to assess and provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities.
• Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities.
• Phone, Fax and Electronic documentation a must.
